Summary of Benefits and Coverage (SBC) Distribution

The federal healthcare reform law, known as the Patient Protection and Affordable Care Act (PPACA), requires employer group health plans to provide enrolled and otherwise eligible individuals with a uniform, easy-to-read “Summary of Benefits and Coverage” (SBC) describing the benefits and limitations of coverage under each plan option.
As part of this federally mandated requirement, the Summary of Benefits and Coverage for all ConVal Health Insurance Plans can be found by clicking the link below.  Be advised that you can request a paper copy of this information by contacting Denise McLenon between the hours of 8:00 a.m. and 3:30 p.m., Monday-Friday.
