There are many benefits that come with your health/dental insurance, so it is important that you create an account with HealthTrust.  Once logged into your HealthTrust account, you will find benefit information such as your plan information, Slice of Life, etc.  You will also find links to SmartShopper, CVS/Caremark, Anthem, Onlife, and Biometric Screenings.  Your Flexible Spending Account claims history can be found by clicking on the FSA Log-in link.  It is also important to create an account with Benefit Strategies to monitor claims and reimbursements.  Click on the links below and follow the process to create your account.

IMPORTANT BENEFIT INFORMATION/CHANGES FROM HEALTHTRUST

  • FLEXIBLE SPENDING ACCOUNT (FSA) (Effective November 1, 2017):  Here you will find the letter from HealthTrust explaining your FSA services.  It is very IMPORTANT that you read this letter carefully as you do need to create a new account:  FSA Letter from HealthTrust
  • ConvenientMD — Site of Service Lab (Effective January 1, 2018):  In addition to Quest Diagnostics for blood work, beginning January 1, 2017, you will be able to use ConvenientMD.  They have many locations, and are open 8 a.m. to 8 p.m., 7 days a week.  EFFECTIVE October 1, 2017 — You can use ConvenientMD for x-rays – ConvenientMD Now Accepting X-Ray Orders, and to complete your Biometric Screening Form – 2018 Biometric Health Screening Authorization Form – ConvenientMD
  • CVS MAINTENANCE CHOICE:  AN ENHANCED WAY TO FILL LONG-TERM PRESCRIPTIONS (Effective January 1, 2018):  CVS Caremark Maintenance Choice
  • SMARTSHOPPER ENHANCED SERVICES (Effective January 1, 2018):  2018 SmartShopper
  • SLICE OF LIFE 2018 ENHANCEMENTS (Effective January 1, 2018):  Slice of Life 2018 – Take a Personal Wellness Journey

Employee contributions for professional and 12-month staff will be taken in 21 pays.  Employee contributions for 10-month staff, and paraprofessionals choosing 16-pays, will be taken in 16 pays.  Below are the Health/Dental Insurance Rates, as well as, the reimbursement information from Benefit Strategies.

The federal healthcare reform law, known as the Patient Protection and Affordable Care Act (PPACA), requires employer group health plans to provide enrolled and otherwise eligible individuals with a uniform, easy-to-read “Summary of Benefits and Coverage” (SBC) describing the benefits and limitations of coverage under each plan option.
